Information about the DES Documents Center Documents

The DES Graphics & Design (G&D) Services maintains the documents. The documents may exist in print, electronically or both. The documents are available to all DES staff and some are available to the public at the public DES Documents Center.

"CS or CSE" followed by a dash and a 3 or 4 digit number and an alpha code for document type.

Example: CSE-1150A

Any DCSS employee may request creation, revision, or deletion of documents by using the Policy Review Request Form. The Policy Unit reviews and approves requests, and will submit a G&D Request to DES Graphics & Design, then track the form request.

- All Title IV_D staff are accountable for proper activity history documentation.

- Activity History must meet safeguarding and confidentiality requirements.

- Be professional and business in nature.

- Be free of slang terminology and unapproved abbreviations.

- Document all case updates

- Document case actions

- Document Employer information and case updates.
